How to create MP4 or MKV files with subtitles added?

Description of problem or question:
How do I rip DVDs of foreign movies that have subtitles so that the resulting MP4/MKV files have the subtitles added?

I am able to rip the DVDs but there's no subtitles included. So when the movie is played, there are no subtitles but just the dialog in the original language.

In the Handbrake window, there is a Subtitles tab. I selected "1 English (VOBSUB)" with the Forced Only and Default checkboxes all selected. But when I rip the DVD, the subtitles are not in the MP4 or MKV file.

Doing something wrong here. Anyone know how to do this or point me to info that explains how to have the subtitles included?
