AUDIO out of sync
on a ripped title the audio starts about 3 seconds late on the final ripped file. this seems odd. Is this a common problem?

I have had this problem too.
When I use DVD Decrypter (Windows), it reports the audio/video offset of a DVD in the filename. So for example, -66ms. So there are, I'm thinking, some DVDs that do have offsets which don't prevent perfect sync when playing the DVD, but do cause problems for us rippers.
Does Handbrake take into account the "natural" offset on a DVD or does it ignore it, thus sewing up the audio and video out of sync at whatever the "natural" offset is?
