dvd episodes not ripping properly

Description of problem or question:

I'm trying to rip tv episodes from a dvd. The first disc and the first two episodes on the second disc were ripped successfully, but I can't rip anymore episodes no matter what I do. I try to label them properly, but they end up being imported with the title grayed out and only a piece of paper symbol next to them. They're not viewable.
