distorted video when ripping DVD

I am using OSX El Capitan 10.11.6 and handbrake version 1.0.3
When I try to convert a DVD into mp4 format, the output video looks very distorted.
Log:

Not sure what I am doing wrong. If anyone can help I would appreciate it. Thank you

We need the encode log that goes with this - should have been in the same location as the scan log.

And an example of the "distortion" would be helpful, as well.
