Maybe it's just plain crazy...
When you export a Keynote presentation (with effects and so on) into a quicktime movie, the result is an interactive movie where you can click here and there and move through the movie clip.
Maybe something could be done to achieve the same for DVD menus, like integrating movie, extras, audio selection etc directly into the movie. Imagine, a simple mov file that looks like a whole DVD structure.
Even simple DVD games could be included in the movie.
On the other side, consider that:
1) I don't have a "touch" device from apple, so I don't know whether an interactive keynote movie would play and interact on them.
2) Anything like that should be accessible from the remote control, so it would be pretty hard!

It's technically possible. But no one will ever implement it probably.

As the others have said, yes, it is possible to have DVD-like menus in MP4 files. The key here is the -like. The way DVD menus are done, and the way menus are done in MP4 are completely different, and translating them would be difficult, if it was possible at all.
DVDs use a Virtual Machine that intrepret a set of instructions stored on the DVD.
MP4 uses either BIFS (Binary Format for Scenes) which is based on VRML, or MPEG-J which is a set of Java classes for use in MPEG.
Matroska, however supports storing DVD menus directly in the container. Not all players support this (in fact, I think VLC is the only one so far), and HandBrake does not in any way.
