Description of problem or question:
I've seen a few dvds that have the main title splitted to 2-3 titles..
Is there no way to get handbrake to understand that and take all three titles to one mp4 or is it necessary to use a third party software to join the three mp3 files?

Not within handbrake.

As mentioned, ffmpeg can splice videos together pretty well, even when they do not match specifications.
Ripping a DVD with actual ripping software, though, you might find that there is a title that contains all the parts. I get a LOT of DVD and BD titles that have multiple "views". MakeMKV shows me what is available on the disk and I can choose what I want, without the delays of waiting for handbrake to encode it. Afterward, I can run that through handbrake, after determining the "best" settings for that disk.
