Visual artifacts/Green Blocks in output files inconsistently

Description of problem or question:
Strange green boxes appear for noticeable moments (.5-1.5 seconds), whole screen freak outs sometimes during fast motion ending credits with text. It is consistent when it where it will show up but not when, i.e. for one series the issues show up at the same points in the introduction or ending credits. For example the entire series of this anime has the issue in all the opening and endings plus random parts have the green box issue.

But for a series that was batch encoded before the screenshotted example the first half of the episodes do not feature the issue but the second half had the repeated artifacts and "broken" sections in the opening/ending/interspersed during the file. The good quality results and the ones with issues were all put into queue yesterday and the machine has been running for about 30 hours pumping through.

This machine is also running a Plex server, was serving direct play and transcoded playbacks, and has a VirtualBox Lubuntu Guest running downloads onto a scratch disk then moving the completed download to the same drive the files being encoded by Handbrake are. So there is random high CPU usage and disk usage on a low end system (Dell Optiplex SFF ex-office PC with an Intel 4core/4thread i5-3470 and 8GB ram). I don't know if this may be a cause or contribution.

Looks like a problem with the QSV decoder. Try disabling it in the preferences to use the software decoder.

I'll keep testing this but it seems to have fixed it, is this a known issue with first generation quick sync processors? Is it configuration or should I just swear off quick sync.

I'm not entirely certain but it does seem to crop up from time to time. QSV decode isn't really necessary, anyway. It will be more useful once we have a full hardware decode/encode path with no software in the way, which is being worked on.
