FWIW: I did discover that MPEG Streamclip does convert the DVD with correct timing. That's a good cross-platform choice for this functionality.

I got a decent encoding using these settings to go from a 720p to aTV:

Video: 944x528, H.264, 29.97fps, CQ 60%
x264 Options: bframes=12:ref=12:subq=7:me=umh:no-past-pskip=1:cabac=0:weightb=1:mixed-refs=1:merange=64:analyse=all:brdo=1:bime=1:no-dct-decimate=1

I'd use MPEG streamclip (it opens DVDs, right?) and export to Quicktime and select an audio-only format.
