Actually, this has just happened to me using Handbrake at width 640. Never happened before. Encode slowed way down on the second pass, result was audio cutting off after about a third of the film (Alien [Extended]). I've discovered that it is possible to extract closed captions as SRT subs. And that there exist methods of converting SRT (and other types of sub) into VobSubs (idx + sub, i.e. pictures rather than text). So if there was an option to include, apart from the subpictures already on the DVD, one's own...

I have had this problem too. When I use DVD Decrypter (Windows), it reports the audio/video offset of a DVD in the filename. So for example, -66ms. So there are, I'm thinking, some DVDs that do have offsets which don't prevent perfect sync when playing the DVD, but do cause problems for us rippers. ...

The funny thing is, I actually live in Region 2 (England). Yet I have deliberately bought many Region 1 DVDs with Closed Captions because their Region 2 counterparts have no subtitles (Yes Minister amongst them).
