Looks like none of them support h.264. H.264 is a MPEG4 codec is it not? It is based on MPEG4. Spec sheets usually list h.264/AVC separately. So, I think I will be waiting it out and seeing if the appleTV sucks as much as I suspect it will (no AC3 sound). As far as I understand it Quicktime mixes down AAC 5.1 to stereo when you output to your awesome 5.1 system, so I can only assume the appleTV will do the same, as it is based on Quickti...

By not supporting CABAC and B-Frames does that mean @TV won't play those videos at all or will it just not turn that stuff on?

What is the point of using Main Profile over Baseline if one is not going to use CABAC or B-Frames?

PCM is lossless audio, sorta like AIFF or WAV. HB/MF have problems with some (not all) PCM tracks. See if you can select a different audio track (ac3). If not you will need another program and/or will have to mux the audio with the video yourself.

Handbrake has problems with certain DVDs and cannot find all the titles. Run the VIDEO_TS folder through dvdoneX (movie only, remove zero cells if there) or rip with MTR 3.0 14r using 'Main Feature Extraction'.

What MTR beta version do you have?

Only 'Main Feature Extraction' and 'Full Disc Extraction' produce a valid VIDEO_TS folder, not title/chapter extraction. Title/chapter extraction will produce one VOB file (at least it did in MTR 3.0 13r).

You need to make a VIDEO_TS folder with only one title. Use DVDoneX (Movie Only) like dynaflash said or MTR beta (Main Feature Extraction) if you already have that. I had the same problem with Hitch (MF/HB would not find the main feature title). There are two ways you might be able to fix this, they both do the same thing which is to create a VIDEO_TS folder with only one title. You can use DVDoneX or MTR beta to do this.
