Custom settings for Decomb are in the Filters UI.
Saving you some time here, compare EEDI2 and Default. Pick one.
There are no better custom settings to be found or contrived.

Handbrake uses the same libraries as VLC, so if VLC can't open it, ...
Use MediaInfo to post your source file properties; someone "may" be able to help you.

If your x265 comes in around 2/3 the size of the x264 original, you're in the right neighborhood. There is no real correlation between CRF / CQ numbers across encoders, and the index disparities get worse with hardware encoders. The numbers pretty much are what they are, all the way from 12-30.

The player specification tells you what file formats and parameters are supported.
Each model is different, so start there, then encode to those parameters.

It may also slightly reduce color banding issues.
it has been visibly demonstrated in tests that when importing 8 bit 4:4:4 or 10 bit YUV original source, banding is reduced over conventional 8 bit dithering, even if outputting 8 bit 4:2:0 for delivery.

AC3 audio has never been very friendly with iPhone, in varying degrees since they were invented.
See if things smooth out with AAC as in the presets. Just something to test.

MakeMKV rips the untouched, pristine Mpeg-2 from your DVD to MKV file, bypassing copy protection for single personal use.

Handbrake converts the MKV to smaller portable mp4 files.
This is not what you wanted to do?
Will you state your need, because I'm not sure what it is.

If you encode "Same as Source" and "Constant" Frame Rate, soft-telecined progressive source will reveal itself intact and "as shot" in the Handbrake render. I'm guessing that interlacing is no longer be done on blu-ray or dvds
MOST DVD and Blu Ray formats are interlaced by the specifications.

Your file encoding looks fine.
I'm sure you know that MP4, M4V, files are MPEG.
iTunes does not produce MPEG-2, so there is another reason you are not seeing video.
Sorry I can't help you past that; iTunes is a classic "black box" to the rest of the world.
