You can play with the CQ slider or bitrates, but as you are aware, you are going to trade either quality or image real estate / bitrate in order to get smaller files. That being said, I do most of my HD transcodes down to 720p. They play better on my computer, and unless you have a TV bigger than 40...

One might expect a "long time supporter" to have enough self-confidence to be able do without the authoritarian posturing. In this kind of footage, the Deinterlace option in HB produces smoother results than Decomb but are going to be softer overall. It's a matter of which works best for your particular situation. 8bpp (red) + 8bpp (green) + 8bpp (blue) = 24 bit true color = 16.7 million colors

I upload dozens of HD videos to Youtube with B-frames on.
So while turning them off may have solved your particular issue, it is not the root of the problem.
You seem to feel entitled to 24/7 instant clairvoyant tech support (between Midnight and 8 am on a Sunday).

Try changing your extension from .m4v to .mp4
Check "Web Optimized" when rendering.
Youtube accepts h.264/mp4 files from Handbrake without complaint or issue.

Try the VideoReDo h.264 Beta version. It uses a time-proven method of removing commercials, re-indexing GOPs, and joining segments without rendering the video.
However, as a beta, h.264 implementation isn't quite ready for prime time, but good enough to give you an idea of what you can do.

iTunes and Quicktime player for Windows are resource hogs. Choppy, out-of-sync playback is very common. Only thing you can do is back off the render bitrate until it smooths out.
