I don't think changing from "slow" to "medium" is going to make a meaningful difference here. The "medium" x265 preset could be about twice as fast as "slow", sometimes a bit more than that. Now there may be some interlaced Blu-ray content, but 4K sources are highly unlikely to be. Drop the decomb filter, it's gotta be slowing you down significantly.

For the same output bitrate, 2-pass average bitrate and constant RF are the same (only a computer could tell the two apart). The difference is, you cannot predict file size when using CRF, but you cannot predict quality when using 2-pass ABR. So that brings up a point I'm not clear on with Average ...

Nope. In HandBrake, your only option for lossless audio is FLAC.

There is no GPL-compatible TrueHD nor DTS-HD MA encoder that I know of, so those two options are out.

Probably a bug or a limitation of either libdvdnav or HandBrake's libdvdnav wrapper. Just use other software to rip, then re-encode the rip using HandBrake if required.

Some H.264 encoders (e.g. x264) can produce great-looking 1080p videos at 1,500 Kbps, depending on the source. But it really depends on the source.

x265 is not a video format (neither is x264), they're software encoders that produce HEVC and H.264 video, respectively.
