I use MTR to rip the dvd's to a hard drive, then I use 72% CRF Constant Quality with H.264 as the video encoding setting. Thats the problem. Your creating a HUGH file. Bu.. if I leave the quicktime window open at its 1080i size and hit encode, I get 1280x720 @ 25fps at just over 5mg bitrate and it plays perfectly on my Apple TV. Does Anamorphic now work correctly? To clarify: it always worked correctly. The problem was Quicktime doesn't respect sample aspect ratios, not clee's patch working incorrectly.
