I've seen similar problem, I guess it was the issue with my DVD source, but the replies says it does happen sometime.
I've tried rip the disc with Makemkv & [Removed Dubious 3rd party products], both worked and video/audio are synchronized, just taking some extra times.

I've experienced it, believe that mostly because the rip protection.
MakeMKV is one greatest option to rip the encrypted discs, also there are some other choices like Any DVD, [Removed Dubious 3rd party products]... etc if you really need to rip contents from a protected disc

As the previous post mentioned, Don't know if there's a windows file association problem, I would recommend to check the windows control panel(Windows setting) --> Apps --> default apps to see if the .mp4 are correctly associated?
