Start the file playing in VLC. Go to a place where you expect forced subs to occur. If you see them displayed correctly while the movie is playing in VLC, go to the head and make a note of which subtitle track is being played. The go back to HB and select that track under your subtitle track menu.

That's what I have done. Select the aTV3 preset, click web optimize and save it as my default preset.

Subler/iTunes has been working fine for me. What version of Subler are you using? Have you done a "Get Info" on the iTunes file and see if the artwork actually there?

What are you using to try and view the attached file? From what I have read, various systems, e.g. Media Center, XMBC, etc need plugins to be able to view the attached artwork.

Codec: AC3 (ffmpeg)
Mixdown: 6-channel discrete

I have an aTV 3 and also use MakeMKV and HB for BR's. No problem. They look great and stream smoothly over wifi. I use High Profile w Web Optimized, anamorphic strict.
