Not an issue, MKVToolNix will gladly remux and combine the streams into an MKV container without re-encoding.

You can probably import the Mini-DV using iMovie, assuming it still supports this feature (it used to). Then you can export to MP4 directly or export to something like Apple ProRes or Intermediate Codec, and feed that to HandBrake.

Short answer: HandBrake does not currently support encoding to 7.1 E-AC-3.

Yes, this should work for the x264 encoder (x265 uses thread pools and the options are different: http://x265.readthedocs.io/en/default/threading.html).

For the most part, there is no need to mess with this.

Change the automatic cropping setting to custom and set all values to zero (HandBrake crops black borders by default).

Vice-versa. As resolution goes up, not quite as much quality is needed perceptually. So Super HQ 720p30 Surround uses RF 17 which is very slightly lower quality (single digit percent bit rate) than Super HQ 480p30 Surround at RF 16. It's not a huge issue, there are no other changes between those two...

Increasing the number of preview frames usually only works in the case auto cropping is too aggressive. All it takes is one frame with non-black(ish) data at the edges to cause it to back off.

DVDs are 480p (NTSC; America, etc.) or 576p (PAL; Europe, etc.). You may want to use the Super HQ 480p30 (or Super HQ 576p25 for Europe) preset. Minor differences to the 720p presets, notably a slight bump in quality. Not enough that I would redo any existing encodes, just for future reference.

Would be interesting to know whether Plex is transcoding or serving the video untouched. If it's not messing with the video at all, iTunes is certainly suspect.

Looks like it supports vanilla MPEG-4 and AAC in MP4. I would probably start with Fast 480p30 and change the video codec to MPEG-4.

You’re basically achieving a constant bit rate with those constraints. Better to employ NLMeans and/or raise RF.

The thing is, ATV 4K *should* support 1080p30 at Level 4.0 buffer rates. It certainly can handle far more. My ATV 3 does as well. Something really fishy about your device...
