Your source file is already fairly compressed hevc. Reencoding this to h264 using QSV is probably not going to be the best quality.

Can you use makemkv to extract the main movie from the ISO.

Then encode that file.

Logs should exist for previous encodes unless you have deleted them.

Depends on what you consider “Maximum image quality”.

Generally the answer is no. As the saying goes… Speed, Quality, Size pick 2. If you want quality and speed you are going to have to put up with a large file, in which case just keep the original!
