Check in your preferences for the file auto-naming, I suspect its overwriting the files. Maybe set auto-naming to something like [source] [title] [chapters].???

Auto-naming would be good but (for me at least) the ideal would be to also send that information through as an additional attribute on the track e.g.
Track 3
- Type: AC3
- Language: English
- Comment: Directors Commentary
