Maybe there's a bitstream header change that's throwing HandBrake off. Can you try the following way to combine the streams? MPEG Transport Streams don't have global bitstream headers, unlike Matroska or QuickTime.

Do you have the same issue if you set the H.264 level to 4.0 instead of 4.1? This will tell the encoder to enforce stricter peak bitrate constraints, and should help in a streaming environment (even cabled). Nah, --cfr can work standalone (it uses the source's detected frame rate, which isn't always 100% accurate, but should be close enough for 99% of what's out there). That's probably a limitation of the preset you're using. Try saving a custom preset and entering 1080 by 1920 as the resolution limits, instead of 1920x1080.

Most likely too many reference and b frames. Try adding ref=5:bframes=5 to the additional options box on the video tab. No, we can pass the full track directly from the source -- the thing we can't do is decode the extra channels, but that's not relevant for passthru. And no, the decoder's limitations apply to all versions, including HandBrake for Windows.

HandBrake's DTS decoder cannot decode all types of additional channels. The only way to get 7.1 output out of HandBrake for this track is to use DTS-HD Passthru.

You don't need Decomb enabled at all for most Blu-rays (at least when encoding the main feature). The number of BD discs with interlaced content is very low. You'll probably want it enabled for special features (especially, but not only, if they're standard definition) though.

I can get video created with only Handbrake using MPEG-4, VP8 and VP9 without the artifacts as well. All signs seem to point to something strange with the conversion using only h.264. Any idea why? Is there a decoder/encoder on my computer that needs reinstalling or fixing? Well this clearly shows ...

The reason it's done that way is because if a 32-bit MP4 were to exceed 4 GB in size, the "index" would be corrupt and the file unusable.

Many of our filters aren't colorspace-agnostic. Support for 10-bit and other colorspaces is feasible (and presumably, on our long term roadmap), but it's work and that takes time :) I thought you have some sort of automatic algorithm that detects such? Or even an option for the user to specify this...
