Probably sub-optimal audio/video interleaving due to the audio or video track encoding faster than the other track on your AMD computer, is my best guess.

Handbrake is passing the TrueHD track to the output (along with the AC3 version MakeMKV created). Not terribly important, but MakeMKV doesn't create an AC3 track out of TrueHD. HandBrake will succeed at reducing file size, but it won't automatically trim out segments without motion. Not sure what software, if any, would do that for you.

IIRC libx265 doesn't let us flag bitstreams as VFR like we do for same as source with libx264; we just set the framerate to that of the source without accounting for any filters, which is not ideal. Not sure how we could best work around it though.

When remuxing with MKVtoolnix, does demuxing first to an elementary stream and then muxing from that help at all?

Probably a side-effect of application sandboxing.

It might be possible to avoid the crash. I wonder if one can build libbluray with flags to disable libaacs and libbdplus support (which is what libmmbd emulates)?

That's already a lot, most of the encoding is actually done on a dedicated video encoding hardware chip that just happens to be on the GPU.

Both are HEVC encoders; x265 will definitely work at 5400x2700; not sure about the NVIDIA-accelerated one but it's easy for you to try to since you apparently have the hardware for it.
